When you have these conditions, you need to find the Statue of Radagon near the Erdtree Santuary Site of Grace.
Starting from the Site of Grace, Erdtree Sanctuary. Simply take the path descending stairs. This will be the path that exits from the building containing the Site of grace and then lead you to some stairs. As you descend, continue down the elevator/lift, keep going down the stairs and you will find the large statue of Radagon.
Keep the statue of Radagon visible and then perform the Incantation Law of Regression. If the statue changes, Noble Broken Mask will invade. Simply defeat them and acquire the Golden Order Flail and the armor set.
