Eastern Nameless Mausoleum is a Location in Elden Ring, added with the Shadow of the Erdtree DLC. The Eastern Nameless Mausoleum is found in the depths section of the Scadu Altus region, southeast of the Ruins of Unte. See Shadow of the Erdtree Map

Eastern Nameless Mausoleum Map

From the Main Gate Plaza Site of Grace in Shadow Keep, head outside to where the burning boats are. When you make it to the bridge, look for the ladder on the left side and use it. Go inside the waterfall to the left, enter the passage, and use another ladder. Inside the small room, hit the empty wall across the painting to reveal a secret area. Follow the stream of water and interact with the coffin. Go outside and use the Castle Watering Hole Site of Grace.

From the checkpoint, head southeast, pass through Ruins of Unte, and go past the Furnace Golem. The mountainside will come into view and you can use another checkpoint here, the Recluses' River Upstream. From here, navigate going south while carefully jumping between platforms. When the path to the left side ends, jump to the right side. Continue heading south until you reach the Recluses' River Downstream Site of Grace. From there, look down on the nearest cliff, and jump down the gravestone steps protruding from the mountain wall. Go onto the first waterfall (do not go down further) and head north in the water. Follow the only path and do not climb any rocks. Turn east before the small waterfall. You will see the entrance to the Eastern Nameless Mausoleum marked by 2 flaming braziers. Eastern Nameless Mausoleum Walkthrough

The Eastern Nameless Mausoleum is a location found in the depths section of the Scadu Altus region. To reach this, you need to use the coffin that's located under the main plaza section of the Shadow Keep. When you reach the bridge section of the main plaza (the area where you encounter the Shadow Militia enemies), check the east side of the bridge to find a ladder that you can climb down to the underground area of the Shadow Keep's main plaza. (The ladder is next to a body that you can loot that has a Silver-Pickled Fowl Foot item.)

After climbing down, you'll see a waterfall next to it, go through the waterfall to find a room that has another ladder. Climb down the ladder and you'll find a small storeroom that contains an item, the Domain of Dragons Painting. Approach the wall on the right and hit it to reveal an illusory wall. Beyond is a pathway leading to a sewage area where you'll find a coffin that you can interact with. Resting in the coffin will trigger a short cutscene and you'll be teleported to the depths section of Scadu Altus, specifically by the Castle Watering Hole site of grace See Shadow of the Erdtree Map.

From the site of grace, head southeast where you'll pass the Ruins of Unte. Follow the winding river to the south until you reach the Recluses River Downstream Site of Grace See Shadow of the Erdtree Map. From there, just to the southeast, take the tombstone platforms down and use them to safely reach the bottom. Be sure to take the platforms to the north on the way down, as taking the southward path will take you to the bottom of the waterfall, rather than above it. Next, go north toward a smaller waterfall, and when close to it, turn east and on through a mess of slimes in a narrow canyon, until you reach the end where you'll find the mausoleum.

By the entrance, there is a Summoning Pool or effigy for players to use for co-op. There is also a Stake of Marika that you can use to respawn by the entrance of the mausoleum in case you die. As mentioned, the location is where you'll fight against a mini boss, so make preparations before you pass through the mist.
