      If you're playing on PC, there's a mod that fixes this item and the Cap of Wrath. It's called 'Cap Of Wrath and Enraging Heart Garb wrath status fix'. Thank you modders for doing the devs' job for them.

        As of Patch 4 the Wrath still does not proc. It happened once then never again for my game.

        I reported the bug and got a reply back stating it would be fixed in an upcoming patch or hotfix. That was before patch 4. So this might be fixed in the next hotfix.

          There is currently a bug where you can constantly replace this with another chest and vice versa and you'll slowly regain all of your hitpoints.

            Seems that all Wrath items are simply broken, or perhaps the Wrath itself - this garb straight up doesn't work, same for cap of wrath.

              So the way it's worded means that every turn a Barb spends in rage you should generate 2 turns of wrath. But I get nothing, not when entering rage and not during rage, so it seems bugged apparently. Which is sad cause this is a GREAT barb item.
