Bloodied Items in Diablo IV are a new item quality used in tandem with the Killstreaks, which debuted in the 12th Season of the game that released on March 12, 2026. What are Bloodied Items?
Bloodied Items in Diablo 4 are a new quality introduced in Season 12, built around the Killstreak mechanic. These items gain unique affixes that scale in strength depending on how many enemies you defeat in a chain. Bloodied gear can appear on armor, weapons, and accessories; each category is tied to a specific affix type. Rampage affixes on armor increase your defensive and offensive stats based on your current Killstreak Tier, Feast affixes on weapons grant combat bonuses that grow with the total number of kills in your streak, and Hunger affixes on jewelry enhance the rewards you earn from Killstreaks.
Additionally, the Item Quality tag for these items is non-exclusive. This means that gear can both have the tag Bloodied and Ancestral all in one item.
How to Acquire Bloodied Items?
There are multiple ways to acquire Bloodied Items in Diablo 4. The simplest way of obtaining Bloodied Items is by purchasing them from The Butcher in Gea Kul in exchange for Fresh Meat. To obtain Fresh Meat, you will have to participate in the seasonal event to become the Butcher and take part in Helltides, slaughterhouses, and in the Ceremony of Slaughter.
Participating in the Slaughterhouses as the Butcher is the best method among the 3 as you can slay large hordes of demons that can drop Fresh Meat for you. To enter into Slaughterhouses, you will need Slaughterhouse Keys to activate and find Slaughterhouses all over. Increase your chances of obtaining Slaughterhouse Keys by putting Smoldering Ashes inside Urns of Slaughter.

Bloodied Item Mechanics
Diablo 4 Killstreak Mechanics
Bloodied Items are directly tied to Diablo 4’s Killstreak system, introduced in Season 12. Killstreaks track how many enemies you defeat in continuously without letting the meter drop. As you defeat more enemies and let the streak grow, you climb through 6 different tiers that grant an increasing multiplier bonus.
These multiplier bonuses depend on what Bloodied Affix you have on your gear. This is then multiplied by the multiplier bonus granted by your streak. On the other hand, losing the streak or failing to continue the meter will result in losing the affix bonus. Players will have to restart the killing once more and continue the streak.
The number of enemies required for each streak are as follows:

Do note that, it is unlikely to reach Fresh Meat and that most players will only get up to 500 to 1,000 kills per activity.
Bloodied Sigils and Bloodsoaked Sigils
Bloodied Sigils are used to increase the difficulty of Nightmare Dungeons, Infernal Hordes, and Lair Bosses, making the run more challenging. Activating one raises the dungeon’s difficulty beyond its chosen level, ensuring that the event is always at least one Torment Tier higher than the base setting. A Bloodied Sigil will have the Relentless Butcher Bloodied Affix, which increases the enemy's strength and adds an additional charge for the Butcher to spawn. Bloodied Sigil events will always be at least a Torment Tier higher than the difficulty that you chose. Additionally, Bloodied Sigils can be obtained by participating in Torment 1 or higher dungeons.
Bloodsoaked Sigils, on the other hand, increases the difficulty of the content to Pit Tier 100. While it may get more challenging, completing the run will grant you great rewards and multiple Bloodied Items. Additionally, both sigils will grant players with numerous Fresh Meats.
Bloodied Affixes
As mentioned earlier, Bloodied Items have new affixes that grant bonuses depending on the current streak you have. There are three Affix categories, each correspond to the type of gear you have. The Rampage Affix can only be found on Armor, Feast are attached to weapons, and Hunger is correlated to Accessories. While there are only three categories, each of the Affix can have a random effect depending on the category possess.
