Robert Accordino shared thisLast month, I had the privilege of attending the White House Fellows Foundation & Association Regional Conference at the George W. Bush Presidential Center. The White House Fellows family is a gift that keeps on giving. I am excited that the program is back and grateful for the work of Sharon C. Kiely, MD, MPM, FACP who leads our alumni organization. 🇺🇸 Hearing from President Bush was deeply moving. He shared stories from before his presidency, reflections on defining moments, and how painting has helped him rediscover himself in the years since. Laura Bush, Karl Rove, Andy Card, and other former administration officials contributed to a conversation that felt both historic and uniquely relevant during these highly partisan times. The timing of this gathering was especially meaningful. As many of us work hard to engage our communities and uphold our national values, this event served as a reminder that public service — done with integrity — leaves a lasting mark.

The greatest gift of those years was getting to know the extraordinary Ken Goulet through our quarterly Board of Directors meetings. Since that time five years ago, Ken has become among the most significant life mentors and served as my executive coach. Our regular sessions together are highlights in my schedule. During them, Ken has been far more than a coach. He is a champion, supporter, sponsor, therapist, role model, and friend -- through every triumph and challenge -- personally and professionally. Chen, MDRobert Accordino shared thisBig milestone: Cartwheel is now the nation’s largest mental health partner for K-12 schools and families. ⭐🎒 I’ve traveled to 20 states in the past six months. I’ve met with governors and legislators, parents and kids, superintendents and school counselors. Kids are struggling to manage stress and overwhelm, make friends, and focus on school. Parents are exhausted and worried. Schools have become a hub and a haven: Delivering direct care, managing crises, educating families, and coordinating community resources. It makes sense! They’re trusted in their communities, and kids spend most of their days in school. But no matter how hard they work, school staff don’t have the time or tools to fully meet student needs. They’re burning out. We started Cartwheel in 2022 to be a trusted mental health partner to schools and families. We deliver the highest quality direct care to students and families via telehealth, and our platform makes it easy for the trusted adults in a child’s life to collaborate on their care. Today, we’re the nation’s largest telehealth partner for schools, growing 300% last year to serve 350 school districts across 15 states. This moment echoes the urgency I felt two years ago, when I wrote about vaccine hesitancy and the dangers of misinformation for Forbes. That message is even more imperative today. I’m resharing my article because restoring trust in vaccines is not just a medical challenge—it’s a national imperative, rooted in my experiences as a physician, a senator, and an advocate for public health. That’s why I’m proud to join the Board of Advisers for the Vaccine Integrity Project at the University of Minnesota's Center for Infectious Disease Research and Policy (CIDRAP), where we’re committed to rebuilding confidence through evidence and open dialogue. Robert Accordino liked thisRobert Accordino liked this💡A question for the CMIOs, quality leaders, and operators in my network. The largest multisite study of ambient AI scribes so far found they saved clinicians about 16 minutes of documentation time ⌚️per eight hours of patient care. That’s real, but it’s a 3% change. It doesn’t explain the burnout improvements those same clinicians report, and it doesn’t come close to justifying the contract on time savings alone. So when a health system says ambient AI “worked,” what did they actually measure? 🤔 I keep seeing the same answer in different words this week: voluntary adoption, clinician trust, documentation quality. Leading indicators, not outcomes. And I don’t think that’s wrong. But I’m not sure we’ve agreed on what the outcomes are.🤷🏽 A few I’d want on the scorecard 📋: ➡️Did panel size or visit capacity change, and did the clinician choose that? ➡️Did coding accuracy move, and did it move in the direction the note supports? ➡️Did anything downstream change for the patient: follow-up completion, referral closure, medication adherence? ➡️What happened to the clinicians who tried it and stopped? (I liken this to the hype around 🩼GLP-1 use, but want to know more about why someone stopped them than continued them - there’s more to that story). That last one gets skipped most often. I’ve sat on both sides of this, as an operator who had to sign the contract and as a clinician who had to trust the note. What are you measuring that you’d actually stand behind? And what did you stop measuring because it turned out not to matter?

And for me, this day isn’t just a day on the calendar. I’m an overdose survivor. There was a time when an overdose could have been the last chapter of my story. It wasn’t. I’m still here. I’m a mom. I’m a wife. I’m a leader. I’m a friend. I’m a person in recovery. And I’m someone who gets to wake up every day and keep writing the story that I almost didn’t get to finish. That’s why overdose awareness matters so much to me. Behind every statistic is a person. A family. A child. A mother. A father. A friend. A colleague. A future that deserves another chance. And behind every survivor is a reminder that recovery is possible. We have to keep talking about overdose without shame. We have to keep expanding access to lifesaving medications, treatment, harm reduction, and recovery support. And we have to stop treating people who use drugs as if they are disposable. Because I know firsthand: An overdose does not have to be the end of someone’s story. I’m grateful mine wasn’t. Today, I remember the people we’ve lost, honor the people who survived, and stand with the people who are still fighting to make it to tomorrow. The learning curve has been steep, but as someone who loves to learn new things, it's also been very exciting. I just returned from an amazing family trip to Ireland and pictured below is my family sitting on the edge of a cliff next to the most beautiful emerald green water. We spent 4 days in Dublin and 3 days on the west coast (Killarney, Dingle). One of the reasons I opened my own practice is so that I could have more flexibility to travel with my family. I'm back, feeling refreshed, and ready to hit the ground running. On that note, if you or anyone you know needs defense or employment counsel, let's connect!

Robert Accordino reacted on thisRobert Accordino reacted on thisThe GLP-1 phenomenon is powerful but it depends on patients staying on the medication longer term to maintain the BMI gains. And what does not get discussed enough is the discontinuation rate. ⚖️ 🏃♂️ A Danish study showed over half of patients who started discontinued within a year. 🪓 Link below. My own experience matched this. I started microdosing Wegovy and then Foundayo just to understand what our patients were experiencing, and followed the titration-up recommended path. I was interested in mild weight loss but also reports of concentration or productivity benefits anecdotally from friends. At low doses, the weight loss or hunger impact really did NOT show... in several months, I only lost 2 pounds. (I was deliberately testing only the medication, not a program, to see the "pill-only" variable.) As soon as I ramped up to a higher dose, immediately I began experiencing REALLY bad diarrhea. Not the "take some psyllium" kind... the wake you up in the night, 10+ times a day episodes. Even the doctor said stop immediately. So I was perhaps part of a large minority of non-responders and then side-effect impacted patients. I was lucky. I could use to lose a few pounds, but I am relatively healthy and not obese, just slightly overweight. This is why based on the data (similar studies in US due to cost, side effects, wariness of permanent medication impacts) I believe we need comprehensive and personalized medical weight loss programs, not just a pharmaceutical-only, one-size-fits-all approach. Accurso, Ph.D.Robert Accordino liked thisWe are proud to share that the University of California, San Francisco will be home to a new national Center of Excellence for Eating Disorders, created to improve early identification and expand access to treatment across the country. Housed in our Division of Adolescent and Young Adult Medicine, the center is supported by a five-year federal grant of more than $9 million from the Substance Abuse and Mental Health Services Administration. Led by Jennifer Carlson, MD, with Sara Buckelew, MD, MPH, serving as curriculum director, the center will train health care providers nationwide, develop screening and clinical tools, create curricula for medical education, and connect experts across institutions to advance eating disorders research and care. By its fifth year, it aims to train more than 1,000 providers annually. We are excited to see our faculty build on UCSF’s strengths in clinical care, research and education to help more people recognize eating disorders earlier and connect patients and families with the care they need. Amy Bonomi, Dean of SDSU's College of Health and Human Services lifted up graduates who take HOPE into their workplaces and family lives. San Diego's Child and Family Well-Being Department demonstrated what it looks like to infuse HOPE into child welfare services. Far beyond "swim and gyms", the YMCA showed what it looks like to saturate whole communities with HOPE. Rady Children's Hospital is showing how a whole state can be infused with HOPE through California Training Institute (CalTrin) and the California Evidence-Based Clearinghouse for Child Welfare (CEBC).
