Michael Anderson, MD, MBA shared thisChildren’s Health Friday: The diseases vaccines taught us to forget There was a time when three letters struck fear into pediatricians: Hib — Haemophilus influenzae type b. Before effective vaccines, Hib caused about 20,000 cases of serious disease in American children every year. It was the leading cause of bacterial meningitis in children under five and could cause sepsis, epiglottitis, pneumonia, permanent neurologic injury—and death. Then something remarkable happened. Vaccination reduced invasive Hib disease in young children by more than 99%. Today, many parents—and even many younger clinicians—have never seen a child critically ill from Hib. That is an extraordinary public-health achievement. But it creates a paradox: when prevention works this well, we can begin to forget what we're preventing. Hib has not become a harmless bacterium. We made Hib disease rare through vaccination. As we confront declining confidence in childhood immunization and the reappearance of vaccine-preventable diseases, we need to remember the lessons our predecessors learned firsthand. Rare does not mean harmless. Preventable does not mean eradicated. Vaccines didn't make these diseases less dangerous. They made them less common. Move on. But that's not what we observed. One of the clearest lessons from WittKieffer's work with the American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) was that leadership growth happens through repetition, reflection, and real-world application. Not a single event. The physicians who gained the most weren't just consuming content. They were receiving feedback, building relationships with peers, working with experienced coaches, and putting new ideas into practice over time. But preparedness for children starts much closer to home—in every emergency department and hospital that may be called upon to care for a critically ill or injured child. Children are not simply small adults. The right equipment, medications, clinical expertise, transfer relationships, and pediatric emergency care coordination all matter. The encouraging news is that many of the same investments that improve everyday pediatric emergency care also make our communities more resilient when disaster strikes. In this commentary pediatrician Annemarie Stroustrup, MD, MPH reflected on the potential impact of the shortages, saying that she has, “been reminded annually for the past several years that my grandchildren will have to wait longer and travel further to receive medical care than my kids.” Improving reimbursement rates, supporting pediatric training, and increasing specialist salaries could help to mitigate the workforce shortages. I was honored to be invited, and this marked my first Corps event since my own retirement from the service last year. I delivered a message of encouragement about remaining steadfast and principled and unafraid, in an era of attacks on public health and public service. Soon, it will be necessary to rebuild the devastated national systems for public health, emergency response, medical research, means-tested human services, and other essential government services that have been gutted. That is not a job for political appointees of either party. It is a job for the professionals, for career federal public servants. The country is waking up to how much it needs public health and career public servants, and that is what I want officers and civil servants still in HHS to hear, and remember. Hold fast.

His argument: By 2030, autonomous AI will outperform physicians — even physicians using AI — at many of medicine’s cognitive tasks. I respect and appreciate Zeke’s views on a variety of issues but on this one, we are going to have to disagree. Zeke may be right about the tasks. But medicine is not a collection of tasks. Getting the diagnosis right matters. Choosing the right test matters. Recommending the right treatment matters. But so does knowing when the textbook answer doesn’t fit the person sitting in front of you. Understanding what a patient values. Knowing when something doesn’t look or feel right and having the judgment to act. It’s why, even with autopilot, most of us would not board a plane without a pilot in the cockpit. Clinical performance is only part of the equation. Performing medical tasks well is not the same as practicing medicine well. Even the JAMA piece acknowledges that much of the evidence comes from simulated tasks, not real-world clinical encounters with real-world patients. That matters when we’re talking about taking the physician out of the equation. Zeke has never shied away from a controversial take. And that promotes discussion - which is what we need. If you’ve read his latest book, you will know he supports eating ice cream. Zeke, let’s revisit this over ice cream in 2030. I like my odds 😎

Michael Anderson, MD, MBA liked thisMichael Anderson, MD, MBA liked thisAs our tech authority meeting stretched into the evening it culminated (as many of our mission meetings do) in a tiny two-letter word; “go”. At NASA, there is a huge amount of weight in that tiny word. Behind it lies intense preparation, intense analysis, and the willingness to stand by your word. In the high-stakes world of human spaceflight, saying "go" is the ultimate expression of professional accountability. As space medicine experts embedded in the launch team, flight surgeons monitor the health and physiological telemetry, environmental systems, and crew readiness down to the second, certifying that the astronauts are ready for the violent physics of ascent. From there, the MMT surgeon evaluates broader operational risks and medical contingencies and gives the MMT chair and launch director a “go”, while the Chief Medical Officer holds ultimate institutional medical responsibility, giving the final medical clearance that carries the collective weight of the entire medical directorate at the flight readiness review. There is a great deal of weight, late hours, argument, toil, analysis, hand wringing, sleepless nights, and work that goes on behind the scenes before someone accepted the full accountability of saying “Go”. When you put your professional reputation and a human life on the line, it stops being an academic exercise and becomes the ultimate test of trust, competence, and courage. In 2009, a group of concerned Greater Clevelanders organized an effort to help the growing numbers of people in the region who were uninsured or underinsured. They ambitiously planned a large-scale event to provide needed healthcare services for 5,000 people over one weekend. While the event itself was cancelled due to the H1N1 flu epidemic, the volunteers harnessed the momentum which had been built and formed Medworks.
