Loot lies in the heart of Diablo 4, and Unique items serve as the ultimate chase for players who want to min-max their characters. The hunt for Uniques has gone through some changes since the game's launch, and in this regard, there's no bigger addition to the game than the ability to target-farm those coveted pieces of build-defining gear.
Torment-tier bosses were added to Diablo 4 in Season of Blood, and they offer players a riveting challenge along with a chance to obtain Unique weapons and armor. Loot drops vary per boss, though, so here's a quick cheat sheet on who drops what.
Updated May 7, 2026 by Marc Santos: The launch of the Lord of Hatred expansion also saw a sweeping change to itemization. Some Uniques were replaced with new ones, which led to a shake up on every Diablo 4 boss loot table. We've updated this guide to reflect these changes, and we've also added new tables for Generic Uniques and other bosses that are yet to be included on this list.
All Lair Bosses can only be challenged in Torment difficulty. We'll be updating this guide constantly as new discoveries are made.

Duriel becomes a farmable boss once in Torment difficulty in the Gaping Crevasse near Gea Kul. However, in order to open Duriel's Treasure Hoard, you'll need to get a Greater Lair Key by opening the hoards of lesser Lair Bosses like Varshan, Grigoire, and Lord Zir. Greater Lair Keys are also needed to open the treasure hoards of Andariel and the Harbinger of Hatred.

Varshan is an excellent source of some of Diablo 4's best Unique items. Players can find him hiding in the Malignant Burrow dungeon next to the Tree of Whispers in Hawezar. You can summon Varshan at any time on any Torment difficulty, but to open his loot box, you'll need a Lair Key.

Grigoire can be found in the Hall of the Penitent in the Dry Steppes, directly south of Ked Bardu. Like Varshan, his treasure chest can be opened by using a Lair Key. Alternatively, you can get a random Unique from Grigoire's loot pool by using the Horadric Cube's Amalgamation function on five pieces of Living Steel.

The Beast in Ice is inside the Glacial Fissure dungeon south of Kyovashad in the Fractured Peaks. You should be able to just waltz right into this dungeon as soon as you reach Torment 1, and yes, you'll need a Lair Key to open the Beast in Ice's loot chest too. If you get any Distilled Fear, bring five of them to the Horadric Cube for a shot at getting one of this boss' loot drops.
Lord Zir
Lord Zir is in the Darkened Way, a dungeon in the northern tip of the Gale Valley region in the Fractured Peaks. It can be found northeast of Yelesna. This is another lesser Lair Boss, and you'll need a Lair Key to open his treasure hoard. Lord Zir's loot can also be obtained from the Horadric Cube by amalgamating five pieces of Exquisite Blood.

Diablo 4 Season 4 introduced Andariel as one of the bosses players can fight in Torment difficulty. She one of the Greater Lair Bosses in Diablo 4, and as such, you'll need a Greater Lair Key to get any spoils from her treasure hoard. Previously, Andariel shared the same loot pool as Duriel, but as of Lord of Hatred, she now has her own set of drops.

The Harbinger of Hatred received the same treatment as Andariel; you'll need a Greater Lair Key to open his boss chest, so you'll have to do a lot of lesser boss farming if you want to get a shot at any of the big bad wolf's loot. You can find this boss in Nahantu, southwest of Gea Kul.

Urivar is a farmable lair boss found in the Fields of Judgment dungeon in Nahantu, just west of the Kichuk waypoint. To summon him, you'll need to get 12 Judicator's Masks, which you can get from Whisper caches, world bosses, and rewards from Kurast Undercity runs.

Astaroth is a boss that players will face at the end of Escalating Nightmare Dungeon runs. To challenge him, you'll need to find and use an Escalation Sigil and complete the subsequent Nightmare Dungeon gauntlet.
Diablo 4 Infernal Horde Loot Table
Infernal Horde was introduced in Diablo 4 Season 5 as a wave-based survival mode. The goal here is to complete all waves and beat the Fell Council without using up all available revives. Collect Burning Aether to open chests and earn plenty of loot. Among the items you can get here are the following:

After completing all waves in Infernal Hordes, you can opt to fight either the Fell Council or Bartuc, a completely different boss within this activity. You'll need 666 Burning Aether to challenge him, so you best make sure that you're clearing those Infernal Hordes waves as quickly as you can.
The Butcher
Players can fight a Lair Boss version of the Butcher in The Broiler, which can be found inside Gea Kul. The Bloodied Butcher is significantly stronger than the variant that players face in dungeons; he has a new moveset as well as some environmental challenges that crank up the danger level. To claim loot from the Butcher's Hoard, players need Pounds of Flesh, which drop from Bloodied activities during Season 12.
As of Lord of Hatred, claiming loot from the Butcher now requires a Greater Lair Key.

Belial, the Lord of Lies
Belial is special among the Lair Bosses. You can find him inside the Palace of the Deceiver in Caldeum, Kehjistan, but instead of having the usual Diablo 4 boss loot table, he gives you the opportunity to get loot from any of the other Lair Bosses. Once you beat him, interact with the chest that appears, then select the boss you want to get loot from. Instead of keys, opening Belial's horde requires two Betrayer's Husks.
Mythic Uniques
Previously known as Uber Uniques, these items are as powerful as they are rare. Mythic Uniques have the lowest drop rates out of any item in the game, but they also come with the best stats and effects in the game. These items can drop from any boss.
