A new AHRQ report, Adverse Events Among Medicare Hospitalizations in 2021–2023, presents national findings from the Quality and Safety Review System. The analysis captures more than 40 types of adverse events through systematic review of inpatient Medicare records from rural, targeted urban, critical access, and other acute care hospitals. Between 2021 and 2023, the share of Medicare patients with at least one adverse event per hospital stay declined from 7.1 percent to 5.2 percent, while the overall rate of adverse events dropped from 83.1 to 59.4 per 1,000 discharges. Across all 3 years, medication events, pressure injuries/ulcers, and hospital-acquired infections represented about 80 percent of adverse events. Critical access hospitals consistently showed the lowest occurrence of adverse events compared with other hospital types. By June 15, Medicare is expected to post a draft decision memo that is likely to address whether to continue Coverage with Evidence Development (CED) requirements for transcatheter aortic valve replacement (TAVR).   A close look at the TAVR CED program and its associated national registry, which has required detailed data collection for 14 years, offers some important lessons about potential improvements to the CED program. Medicare's CED program was designed to provide early access to promising technologies while filling evidence gaps through structured data collection. For TAVR, a minimally invasive heart valve procedure, the CED linked to data collected in the Transcatheter Valve Therapy Registry has supported tremendous improvements in patient outcomes:  with over 500,000 procedures documented in the registry, 30-day mortality decreased by nearly two-thirds, and the TAVR registry infrastructure became a national model. But the data collection mandate has now been extended well beyond the time necessary to address the critical uncertainties identified when CED was initially implemented for TAVR.  And the CMS data collection mandate has some downsides: registry participation is expensive for hospitals, and the requirements may be limiting access in rural and lower-income communities. In a new commentary, CEVR's Paige Lin, Sean Tunis, and Peter Neumann argue that CMS should modernize CED with four principles: time-limited mandates, pre-specified exit criteria, indication-level stratification, and targeted data collection once mandates end.
