As pressure grows to align spend with value, the next chapter of value-based care is coming into focus. The first chapter of value-based care focused on both topline and bottom-line levers; however, the topline lever (risk adjustment) seemed to be pulled more often. In the chapter we’re entering, medical cost management will be front and center. At our Annual Meeting last month, a panel of healthcare leaders shared lessons from our VBC journey so far and offered perspectives on the path ahead. Moderated by Ellen Herlacher, this panel included Ali Khan, MD, MPP of Aetna, a CVS Health Company, Bennett Graham of Main Street Health, Sylvia Hastanan of Greater Good Health, and Anthony Keck of Ballad Health. A few takeaways from the panel included: - The early innings of VBC have shown promise. We’ve seen a basic validation of the VBC principle: Populations in value-based contracts have better outcomes and lower costs. - Much more must be done to confront current and future pressures on the healthcare system. The cost structure of care delivery must come down, points of misalignment between healthcare constituents must be ironed out, and contracting must more accurately incent and capture value creation. - Innovations in AI and automation have the potential to lower the cost structure of care delivery by more precisely assigning therapies and clinical resources based on acuity. - There are many provider re-orchestration and administrative levers to pull that are less tech-intensive, yet still fairly innovative. Among those are business models and attribution frameworks that transition providers away from pure fee-for-service into more value-oriented modes of care. In addition, there are alternative forms of staffing and “extender” models that help expand access to care in rural and underserved communities. They’ve taught me more than they know; and made the hard moments lighter and the sweet moments brighter. We’ve taken big swings, navigated plenty of change, and stayed anchored by what matters most: the customers we serve. Bennett Graham liked thisBennett Graham liked thisRural America doesn't need another pilot program. It needs a proven model. Nearly 90% of rural America sits in a healthcare shortage area. Most solutions I see are workarounds: virtual-first models, or a thin layer of support wrapped around a clinic already on the brink. Almost none of them solve the hardest part, which is connecting patients with a dedicated physician, nurse practitioner, or PA who actually knows them. That's where we started. We directly employ our clinical teams, back them with modern tools, and now serve more than 15,000 patients across 12 clinics in Western North Carolina. Our clinics reach profitability in roughly 14 months, proof that rural primary care can work, and work well. A few days ago we announced our $53 million funding round, which isn’t simply an inflection point for Hopscotch, it’s a catalyst for what’s to come.
