𝗖𝗠𝗦’ 𝗔𝗖𝗖𝗘𝗦𝗦 𝗠𝗼𝗱𝗲𝗹: 𝗔 𝗖𝗼𝗺𝗽𝗹𝗶𝗰𝗮𝘁𝗲𝗱 𝗜𝗱𝗲𝗮 𝗧𝗵𝗮𝘁 𝗠𝗶𝗴𝗵𝘁 𝗙𝗶𝗻𝗮𝗹𝗹𝘆 𝗠𝗼𝘃𝗲 𝘁𝗵𝗲 𝗡𝗲𝗲𝗱𝗹𝗲 CMS just rolled out the new ACCESS Model, a 10‑year experiment that does something Medicare rarely does: it pays providers based on whether patients actually get better.    Not whether they checked the box.  Not whether they documented the right codes.  𝘐𝘵’𝘴 𝘢𝘣𝘰𝘶𝘵 𝘢𝘤𝘵𝘶𝘢𝘭, 𝘮𝘦𝘢𝘴𝘶𝘳𝘢𝘣𝘭𝘦 𝘪𝘮𝘱𝘳𝘰𝘷𝘦𝘮𝘦𝘯𝘵.   Is it complicated? Absolutely. But that’s healthcare. And this might be the right kind of complexity.   The playbook for improving chronic disease is clear. Personalized support. Digital tools. Care coordinators. Starting Jan 2026, health plans will be required to: ↳Resolve urgent PA requests in 72 hours, standard requests in 7 days ↳Publicly report approval/denial rates and turnaround times By 2027, plans must also implement FHIR-based APIs for prior authorization, payer-to-payer exchange, patient access, and provider access. I often hear speed and data integrity framed as a trade-off in healthcare data transformation. That hasn’t been my experience. Population-scale EHR-to-FHIR conversion doesn’t need to take weeks or months, and it doesn’t require compromising on validation, auditability, or governance. With the right foundations, cloud-native architecture, AI-assisted mapping, and governance built in by design, it’s possible to transform 1M patient records into FHIR in hours, not weeks.
