It fails through small overrides. A plan changed after it's already been agreed. Credit for an idea landing one level up. A decision reversed without the person who owned it being told why. None of these feel like betrayals in the moment. Each one is small enough to explain away. But ownership isn't a one-time investment. It has to be maintained, or it decays quietly while everything on paper still looks fine: attendance, output, sign-off.
