Omensmirk Mask protect the player's head by applying various defensive properties, it also changes the appearance as well when it is equipped. Some armor pieces may be available to both genders but may be slightly different for male and female characters.

AnonymousEven with the release of the DLC I find myself wearing this mask for the amazing Poise-Weight ratio and relevant +2 Strength stat boost. The Circlet of Light gives 1 less Poise and the stats it provides are harder to take advantage of for most builds, meaning that even with it's lower weight this mask is still the best mask in the game. It's also fashionably ugly compared to some waifu character wearing the Circlet of Light, if you are going to min-max then you may as well look as monstrous as possible.

AnonymousMaybe the best poise to weight ration of any helmet in the game AND increases strength by 2 points. Put this on with Exile Armor, Tree Sentinel Greaves and Vulgar Milita Gauntlets to get 51 poise and a decent look. Then you can then switch in the Black Knife Armor to become a stelathy assassin while keeping 51 poise.

Anonymous"Elder" could also be a term referring to a person in a position of power, or a father/grandfather figure. So basically, there is an actual Father Mask in this game, if you squint hard enough.

Look up Bloodborne's Loran Clerics. This might explain the missing horn on the nose too.

This kinda outlandish, but what if the mask is modelled after the Dung Eater's "true" face, or soul. We know he has "the heart of an omen without a body to match", and he wishes to see himself and be seen as an omen, that's why he puts those horns in his armor. And indeed, if many (or all) omen are a result of the DE's defilement, it means that they were, in a past life, murdered and defiled by him, which may very well cause them horrible nightmares in their new and accursed life, like PTSD.

This might be the strongest Helm in the game, at least if you need Strength and Poise. 6 Poise for 3 Weight is the best poise-to-weight ratio you can get for a Helm, and it even gives you +2 Strength on top of that for no downsides. Also, it's nasty ugly (in a good way.)

AnonymousUsing this I can get higher poise breakpoints that just aren't possible with the imp mask. It's a shame because I spent all that time farming the imp mask just for this thing to turn out better.

The only elder I can think of is the imp head. "Resembling an elder, it holds trace amounts of arcane knowledge."
